Albany Primary Care will be called Praxis Health Albany. Your provider, your care team, and your medical records are all staying exactly the same. The only thing changing is our name.

Our clinic has been part of the Praxis Health family for years. You may have noticed the name on our logo, in a newsletter, or email. Starting December 1, we are simply making it official.

Praxis Health is a network of independent, provider-led clinics across Oregon, Washington, and Idaho. We are not a hospital system. We are not backed by private equity. We are an independent family of community clinics, each with their own history, now going by one name so that you can find us, recognize us, and know what we stand for.
